골격성 Ⅲ급 부정교합자의 체질량지수에 따른 술후 연조직 변화

Abstract
결론
본 연구에서는 대조군으로 정상교합자 38명을 BMI에 따라 저체중군 정상체중군 및 과체
중군으로 분류하고 각각 안면 연조직의 두께를 측정하고 술후 그 차이의 유의성을 분석하였
다. 실험군으로는 1995년 1월부터 1998년 3월까지 본과로 내원하여 골격성 Ⅲ급 부정교합으
로 진단받고 하악지 시상분할골절단술을 시행받은 후 최소 6개월 이상 경과한 32명의 환자
들을 BMI에 따라 저체중군, 정상체중군 및 과체중군으로 나누고 측정한 연조직 두께를 정
상교합자와의 비교하고 술후 변화에 대하여 연구를 시행한 결과는 다음과 같다.
1. 정상교합자에서 정상체중군과 저체중군과의 비교시 모든 안면 연조직의 두께가 정상체
중군에서 더 두꺼웠고 과체중군과 정상체중군의 비교시에는 G', Ss, Me'에서 과체중군의
연조직이 더 두꺼웠다.
2. 골격성 Ⅲ급 부정교합자는 정상교합자와 비교할 때 연조직의 두께는 하악의 계측점, 즉
Li, Ls, Pg', Gn', Me'에서는 더 얇았고 Ss, Ls에서는 보다 두꺼웠으며 G'과 Ss, Ls에서는
차이가 없었다.
3. 수술직후 각 군에서 연조직 두께는 Ls에서 감소되었고 Li, Pg', Gn', Me'에서는 증가
하였으며 과체중군에서 연조직의 두께는 Li, Gn', Me'에서 상대적으로 크게 증가하였다.
4. 술후 6개월 경과 후 실험군 모두에서 연조직 변화는 Ss, Ls에서는 두께가 감소하였고,
Li, Pg', Gn', Me'에서는 증가하였으며, 특히 과체중군에서는 Gn', Me'에서의 연조직 두께
증가가 상대적으로 컸다.
5. 골이동량에 대한 각 군별 연조직의 반응 정도를 비교하기 위해 Pg'에서 수평이동량을
측정한 결과, 수술직후 저체중군이 91%, 정상체중군이 87%, 과체중군이 81%였으며 술후 6
개월 경과후에는 각각 96%, 91%, 84%로 BMI가 낮을수록 골이동량에 대한 연조직 반응이
높은 것으로 나타났다.
이상에서 안면 연조직 두께는 BMI와 밀접한 관계가 있으며 BMI를 안면연조직의 양을 객
관화하기 위한 간단하고 유용한 임상지수로 이용할 수 있다는 것과 이를 통해 술전에 환자
의 체형에 따른 안면연조직의 술후 변화를 보다 객관적으로 예측할 수 있다는 결론이 도출
된다.
This study has been carried out in order to measure the thickness of soft-tissue on
lateral cephalograhphs based on body mass index(BMI) and the change in soft-tissue
thickness after surgical correction of mandibular protrusion. The control material in
cephalometric study comprised students at The Dental College, 38 persons, aged 21∼24
years and the patient material comprised 20 women and 12men, aged 19∼28 years with
mandibular protrusion. The thickness of the soft-tissue based on BMI in control and
study groups, the comparison between them, immediate postoperative change in the
thickness, 6 months after surgery, ratio of soft-tissue response and correlation was
established through various statistical methods.
The result were as follows :
1. The groups based on BMI showed significant differences each other as regards the
linear measurements.
The thickest soft-tissue was measured 13.6㎜, 15.47㎜, 16.76㎜ at Ss, the thinnest at
G' 6.0㎜, 6.7㎜, 7.26㎜ respectively
2. The differences between control and experimental groups based on BMI showed to
be significant. There were no differences at G'. The sort-tissue in prognathic patients
was thicker at Ss, Ls and thinner at Li, Ls, Pg', Gn', Me'. Differences gap was greater
in overweight groups.
3. The immediate soft-tissue change after surgery showed the increase at Li, Ls, Pg',
Gn', Me' except G' Ls in all groups.
4. The postoperative soft-tissue change 6 months after surgery was similar with
immediate change. The soft-tissue shows the increase in the thickness at Li, Pg', Gn',
Me' and the greatest difference occurred at Li, 1.1㎜, 0.98㎜, 1.2㎜ respectively.
5. The patients with lower BMI index showed higher soft-tissue response to bony
movement at Pg' The immediate response ratio was 91%, 87%, 81% in A, B, C groups
respectively, the response 6 months after surgery showed 96%, 91%, 84%.
